Record of Divorce or Annulment Form

The Record of Divorce or Annulment form (DCH-0838) needs to be sent to the Office of the State Registrar to be added to the State vital records system.  

Constituents often request certified copies of the Record of Divorce or Annulment to obtain a Real ID, a passport, dual citizenship, or claim a pension.  It is also used for re-marriage, genealogy, or issuance of a Social Security Card with a new name.

According to Michigan law (MCL 333.2864), the petitioning party or their attorney must provide the form to the clerk of the court.  When the divorce or annulment is finalized, the clerk of the court will need to complete the record and mail in the form.
